To participate in District testing this year, all families NEED to complete a one-time registration with Color/Valencia Lab for the testing process. Please complete a registration for each student in your family, connected to their school of attendance.

Students at Fisher may come to the testing site without their parent IF the parent completes this form and sends it with their student for the initial/first time testing with the District in 21-22. Please note that the testing permission is ongoing, and you do not need to resign the permission each time. Also, note that the one exception to testing during class time is voluntary testing with band and choir students on Monday each week.

Home COVID Testing:
Based on the guidance from the Santa Clara County Department of Public Health, results from the home tests cannot be used for students on modified quarantine or those who have stayed home or been sent home with COVID symptoms. Please attend our daily clinics, seeks testing from your healthcare provider, or schedule an appointment with any of the Santa Clara County free testing sites when your child is ill or on modified quarantine.
